Understanding Your Medications — Compounded vs. Commercial Pharmacy
Some of the medications we prescribe come from compounding pharmacies. Here’s what that means, why it matters, and what questions to ask.
What is a compounding pharmacy?
A compounding pharmacy creates customized medications for individual patients based on a provider’s prescription. Compounding allows us to adjust doses, change delivery forms, remove allergens, or combine ingredients in ways that aren’t available in commercially manufactured products. Compounding pharmacies are licensed by their state board of pharmacy and operate under federal USP quality and sterility standards.
What is a 503A compounding pharmacy?
A 503A pharmacy is a traditional patient-specific compounding pharmacy. They compound medications based on individual prescriptions from a licensed provider and are regulated at the state level. They must comply with USP standards for sterile and non-sterile preparations. Stellar Wellness works exclusively with licensed, reputable 503A pharmacies, many of which hold PCAB (Pharmacy Compounding Accreditation Board) accreditation — the gold standard for compounding quality.
Are compounded medications FDA-approved?
No — and it’s important to understand what that means. The FDA approves specific drug products manufactured under standardized conditions. Compounded medications are prepared individually for each patient and are not subject to the same pre-market approval process. This does not mean they are unsafe — it means they haven’t undergone the same large-scale clinical trials as commercial drugs. We use compounded medications when they offer a clinically meaningful advantage over commercial alternatives and when sourced from reputable, accredited pharmacies that maintain rigorous quality standards.
Why would I need a compounded medication instead of a commercial one?
There are several clinical reasons we may recommend a compounded preparation: the dose you need isn’t available commercially; you need a different delivery form such as a topical cream instead of a pill; you have an allergy to a filler or dye in the commercial product; the commercial version has been discontinued or is on back order; or a combination that doesn’t exist commercially would benefit your treatment. We always use the most appropriate option for you clinically.
What types of medications at Stellar Wellness might be compounded?
Compounded preparations we commonly prescribe include bioidentical hormone therapy in customized doses and delivery forms, GLP-1 receptor agonists for weight management when commercially unavailable, and specialty therapeutic compounds as clinically indicated. We will always explain why a compounded preparation is recommended for your specific situation and answer any questions before prescribing.
How are compounded medications delivered?
Compounded medications are shipped directly to you from the compounding pharmacy, typically within 5–10 business days of the prescription being sent. They arrive with detailed instructions for use and storage. Some preparations require refrigeration. You should never share compounded medications with others, as they are prepared specifically for you.
Can I use my regular pharmacy for my Stellar Wellness prescriptions?
For FDA-approved commercial medications — yes, absolutely. We send prescriptions electronically to the pharmacy of your choice. For compounded preparations, we work with specific licensed compounding pharmacies that meet our quality standards and will send the prescription directly on your behalf.
What if I have questions about my medication after I receive it?
Message us directly through the patient portal. We respond to clinical questions within 1–2 business days. If you experience a severe reaction or any emergency, call 911 or go to the nearest emergency room immediately.
